Five items were combined to create an index of social isolation. One point was assigned for each item. Average scale ranging from 0 to 5, with higher scores indicating higher levels of social isolation.

b:  were not currently married (never married, separated, divorced, widowed)
c:  never or rarely talk on the phone or get together with friends
d:  never or rarely talk on the phone or get together with relatives
e:  did not volunteer or engage in social/ community activities (social clubs, resident groups, or religious groups).
